| A tort claim occurs when a nongovernment party seeks reimbursement for the costs of damages, death, or
injuries if a government operation is belived to cause the loss. Sometimes a government employee is considered a nongovernment party. SF-95, Claim for Damage, Injury, or Death, commonly called a tort claim form is filled out and submitted with all supporting documentation to: Department of Commerce Office of the General Counsel (Tim Conner) Room 5890, HCHB 14th and Constitution Ave Washington, DC 20230 Phone: 202-482-1067 The supervisor of the government operation causing the claim provides accounting code information to the Department of Commerce. |
