Tort Claims
A tort claim occurs when a nongovernment party seeks reimbursement for the costs of damages, death, or injuries if a government operation is belived to cause the loss. Sometimes a government employee is considered a nongovernment party.

SF-95, Claim for Damage, Injury, or Death, commonly called a tort claim form is filled out and submitted with all supporting documentation to:
